Safety Materials Request Form
Effective immediately, all safety materials requests shall be processed through Safety and Health Services, San Francisco Home Office.
Please complete Safety and Health Services Safety Materials Order Form with numerical SCIF# of safety material listing.
All safety materials cost $1.00 each. A $50.00 minimum order (per order form) is required. All orders placed must be prepaid prior to shipping. A check payable to State Compensation Insurance Fund must accompany each order form and be sent to the following address:

Request for safety materials catalog and safety materials inquiries/questions, please contact Safety and Health Services main telephone number at 415-975-2400. Ask for the Safety Materials Coordinator.
Video Library Request Form
State Agencies will send their request directly to Safety & Health Audiovisual Library. Please use and complete the State Contract Audio-Visual Request Form when making your request. The use of video is free and State Fund will provide an AV Catalog also for free.
AV tapes will be mailed at least one week prior to show date. AV tapes are due back seven days after the show date. No more than three AV tapes may be held by the departmental location at any one time.
Any departmental location having more than three AV tapes will not be sent any additional AV's. Audio visuals must be returned in bubble-wrap, padded envelope or secure box to prevent damage. All borrowed videos or DVDs must be shipped UPS Ground or US Postal "PRIORITY" mail and be insured for $400.00 each. The borrower assumes full responsibility for replacement costs for all lost videos. State Fund will bill State Agency for lost videos.
All AV requests must include the following: State department name, responsible contact person and phone number, and show date.
Additional requests for AV's will not be sent out, until the outstanding ones are returned.
