1. Completely review this package.
2. Unit Manager shall submit an email request to SCIF’s generic mailbox: states@scif.com 
   Subject: Request for SCIF On-Line Services.
   • Agency Code - Division Code, if applicable
   • Department Name and Division Name, if applicable
   • Department Address and Division Address, if applicable
   • Name/Email Address of Information Systems Representative
   • Name/Email Address of Security Firewall Administrator
   • Name/Email Address of WC Unit Manager
   • User Information
     • User code: N=New D=Delete C=Change.
     • Service type: S=SOLCA.
     • User Name and Working Title.
     • User telephone and email address.
   • List of Adjusting SCIF Offices
     • TN – Commerce
     • TQ – Riverside
     • TY – Rohnert Park
     • TL – Sacramento
     • NH – Eureka
     • TX – Oxnard
3. Provide a Windows-based PC with Internet access.
4. State Fund will contact your agency for the Vandyke and user authentication software installation procedures.
5. The State Fund will provide a private token key fob for SOLCA access with each user seat licensing installation.
6. The State Fund will provide a start-up package with instructions, including a secure LOGON and PIN#.
7. User instructions will be provided by State Fund.
8. State Fund will issue an invoice for each SOLCA installation as follows:
   $57 for a single seat license and $75 per token key fob.
