How to Obtain Coverage from State Fund *

You may apply for workers' compensation coverage in one of two ways:

• Through a District Office: Contact your local State Fund District Office.
• Through this Web site: Go to Request an Application for Insurance where you can obtain an application by download or mail. The completed forms must then be submitted to a State Fund District Office.
